| The bucket of an aerial lift has gotten caught in the
protective netting around a building. The worker is standing on the handrail of
the bucket in an attempt to climb onto a beam so he can release it, and is
trying to attach his safety belt to the wire on the beam. |

When trying to attach his safety belt to the wire, the worker's foot slipped and he fell from the bucket.

| Tips for preventing similar accidents
|
| 1. |
Do not get on/off of equipment in elevated positions. |
| 2. |
If you must get on/off of a piece of equipment, ensure this can be done safely and use a safety belt. |
| 3. |
Use two safety belts (connected to two locations). Before unhooking the safety belt being used in the bucket, attach the second safety belt to the parent net. |
