| After excavating a pile hole using an excavation auger, a
worker tries to hang the pile for a temporary placement. Because the site has a
weak foundation, the machine is placed on steel support plates. |

Because a part of the crawler of the boring machine slipped
off the steel support plates, the boring machine began to tip. The operator,
sensing the danger, tried to get out, but did not make it in time and was
crushed beneath the machine.

| Tips for preventing similar accidents
|

When using a boring machine on a weak foundation, place steel
support plates properly so that the crawlers and outriggers of the machine will
not slip off of them. |
| 2. |
Before bringing the pile to the hole, ensure that the crawlers of the boring machine are positioned appropriately on the steel support plates. |
| 3. |
When performing pile driving work, assign someone to serve as a guide, and ensure that workers are completely familiar with all planned work procedures. |
